I am having irregular periods at an interval of four to five months. During the first two days of my periods, I suffer from severe cramps. I am 15 years old. Please suggest a remedy. I am a 26-year-old man, married for the last two years. My wife has not been able to conceive. What kind of tests should we do now? For the past four months I have been suffering from severe pain and heavy bleeding with lot of clots during periods. What may be the reasons? Please advise. My daughter-in-law has been diagnosed with cystic ovaries. She has been prescribed to take Ovral L and Dexorange. But after taking this pill regularly she has developed a tendency to vomit. On an average she vomits 14 to 15 times a day. I want to know whether she can conceive or not. Please advise. I am a 37-year-old man, married for the last five years. My wife has been diagnosed to be physically fit for conception. The doctor has told me that I have no sperm in my semen. Please suggest a solution.

Irregular period

I am having irregular periods at an interval of four to five months. During the first two days of my periods, I suffer from severe cramps. I am 15 years old. Please suggest a remedy.

It could be a case of polycystic ovary. You need to reduce your weight if you are obese or over-weight. Keep a record of your menstrual history. Clinical examination and investigations like ultrasonography, blood tests and hormonal tests will help in a proper diagnosis. Surgery is not necessary in all cases. Treatment depends on diagnosis. Please consult a gynaecologist at the earliest.

Fertility test

I am a 26-year-old man, married for the last two years. My wife has not been able to conceive. What kind of tests should we do now?

In your case, it is essential to know the history of sexual intercourse (regularity and difficulty, if any). Past history of tuberculosis (if any) should also be looked into. You need to analyse your semen for sperm count, motility of sperm, morphology or any kind of infection in semen etc. Smoking or tobacco addiction can also make a person infertile. If both are physically fit, then counselling is required. But, if your wife’s menstrual history is irregular, then she needs to get some tests done to determine the exact cause. Both of you should consult a gynaecologist who specialises in dealing with infertility problems.

bleeding with clots

For the past four months I have been suffering from severe pain and heavy bleeding with lot of clots during periods. What may be the reasons? Please advise.

Heavy bleeding associated with severe pain may be due to endometriosis (endometrial tissue outside endometrium cavity of uterus) and fibroid (benign tumours that consist of fibrous and muscular tissue) in uterus. Clinical examination and certain investigation like ultrasonography are needed for proper diagnosis and treatment. Consult a gynaecologist at the earliest possible.

vomitting

My daughter-in-law has been diagnosed with cystic ovaries. She has been prescribed to take Ovral L and Dexorange. But after taking this pill regularly she has developed a tendency to vomit. On an average she vomits 14 to 15 times a day. I want to know whether she can conceive or not. Please advise.

A combination of oestrogen and progesterone pills sometimes stimulates vomiting tendency. She should ask the doctor to change the pill. Polycystic ovary generally causes infertility. But weight reduction and proper medication can cure it. Ovulation inducing drug like clomiphen citrate may give better result. Her husband should also be examined properly.

You are suffering from azospermia (no sperm in semen). The type of azospermia should be investigated by an andrologist. Clinical examination of your sexual organ and biopsy of the semen is necessary for proper diagnosis. Smoking or tobacco addiction can reduce sperm production. Operations like vasectomy, hydrocele and hernia can also lead to obstructive problems. You can go in for ICSI (Intra cytoplasmic sperm injection), where sperm is extracted from the testis for fertilisation. Another option is artificial intra-uterine insemination with donor’s semen. Success rate is about 30 per cent.
